El vocabulario en los niños con Trastorno del Desarrollo del Lenguaje (TDL) o con Retraso del Lenguaje (RL) es más pobre que en los niños con un desarrollo típico del lenguaje, por lo que se pretende conocer qué diferencias existen entre ellos. Han participado 66 infantes entre 3 y 5 años; se ha valorado que vocabulario conocen, se ha determinado que tienen menos vocabulario (52%) que sus homónimos (69%), a excepción de la categoría de transportes, en la que tienen los (...) mismos resultados. Finalmente, se determina el listado de categorías que más necesitan trabajar los niños con RL o TDL. (shrink)

Background: The high level of satisfaction of users of a health service is largely due to the fact that they receive excellent care from healthcare professionals. Compassionate care is an essential component of excellent care. But what do nurses understand compassion to be? Research objectives: To analyse the concept of compassion from the perspective of nurses in the Andalusian Public Health System, Spain. Research design: This is a qualitative study following the grounded theory model. Four focus groups and 25 in-depth (...) interviews were conducted. Participants and research context: A total of 68 nursing professionals working in the Andalusian Public Health System (Spain) participated. Theoretical sampling was used, with participants being recruited using the snowball technique. Ethical considerations: This research was approved by the Research Ethics Committee of the Centro-Almería Health District (CEICA 27/9/17). Findings: From the analysis of the data, four themes emerged that helped to understand the concept of compassion according to nurses: ‘Negative perception of the term compassion’, ‘Compassion and empathy as synonyms’, ‘Beyond empathy’, and ‘Effects of having a compassionate attitude’. Discussion: Nurses perceive the concept of compassion differently to each other and even contradictorily. This concept is imbued with cultural elements, which adds confusion to understanding it, and is even perceived as something negative similar to pity. Conclusion: Nurses confuse the concepts of empathy and compassion as if they were synonymous. Before considering training in compassion for healthcare professionals, it is essential to clarify the concept of compassion through educational interventions. (shrink)

The accuracy of proxies when they interpret advance directives or apply substituted decision-making criteria has been called into question. It therefore became important to know if the Andalusian Advance Directive Form (AADF) can help to increase the accuracy of proxies' predictions. The aim of this research was to compare the effect of the AADF on the accuracy of proxies' predictions about patients' preferences with that gained from informative and deliberative sessions about end-of-life decision making. A total of 171 pairs of (...) patients and their proxies were randomized to three groups. The control group's answers to the Life Sustaining Preferences Questionnaire (LSPQ) were compared with their proxies' answers to the same questionnaire. In one intervention group, the patients had already completed the AADF and given it to their proxies, who used it to guide their own answers to the LSPQ. In the second intervention (discussion) group, both patients and proxies attended two educative sessions guided by trained nurses and later filled in the LSPQ. Comparisons of accuracy and other variables showed a strong association with the discussion group. The findings show that promoting communication between patients and their proxies improves the accuracy of proxies' predictions much more than isolated use of the AADF form. (shrink)

Objective: To measure the stability of life-sustaining treatment preferences amongst older people and analyse the factors that influence stability. Design: Longitudinal cohort study. Setting: Primary care centres, Granada (Spain). Eighty-five persons age 65 years or older. Participants filled out a questionnaire with six contexts of illness (LSPQ-e). They had to decide whether or not to receive treatment. Participants completed the questionnaire at baseline and 18 months later. Results: 86 percent of the patients did not change preferences. Sex, age, marital status, (...) hospitalisation, and self-perception of health and pain did not affect preferences. Morbidity and the death of a relative did. Conclusion: Stability of preferences of older persons in relation to end-of-life decisions seems to be more probable than instability. Some factors, such as the death of a relative or the increase in morbidity, can change preferences. These findings have implications for advance directives (ADs) and advance care planning. (shrink)

Research suggests a direct negative relationship between peers’ unethical behavior and employees’ ethical intention. But several possible mechanisms might explain this relationship in more detail. For example, Machiavellianism is a personality trait characterized by interpersonal manipulation and the use of unethical means to achieve certain self‐interested ends, whether useful or pleasant. This article adopts an Aristotelian understanding of philia, related to three goods on which human relationships rest: useful, pleasant, and honest. We propose that Machiavellianism, a self‐interested, pragmatic personality orientation, (...) might explicate the relationship between peers’ unethical behavior and ethical intention. The results of a structural equation model applied to a sample of 436 banking employees in Spain reveals that Machiavellianism partially mediates the relationship between peers’ unethical behavior and employees’ ethical intention. We also find that with a greater level of peers’ unethical behavior, the negative effect of Machiavellianism on ethical intention increases, and that when peers’ unethical behavior is nonexistent, the negative effect of Machiavellianism on ethical intention disappears. These findings advance current literature by revealing that unethical peers can indirectly influence ethical intention, through shaping Machiavellianism. Our study is also the first to show that pairing high Machiavellians with ethical peers can help to cancel out the negative influence of Machiavellianism on ethical intention. (shrink)

This essay aims to broaden the understanding of the nature of the physician–patient relationship. To do so, the concept of medical philia that Pedro Laín Entralgo proposes is analysed and is considered taking into consideration the relational trait of the human being and the structure of human action as a story of the permanent tension that exists between freedom and truth, where the ontological foundation of the hermeneutic of the "Gift" and the analogy of “Love” as the central dynamic of (...) this action, helps explain the nature of the doctor-patient relationship as a "friendship relationship". This understanding offers a perspective differing from just the utilitarian considerations of current "patient-centred" approaches and proposed the redirection of the teaching of medicine to more humanistic approaches. The dynamic of the doctor-patient relationship proposed here in its most genuine essence is effectively expressed in physician concrete attitudes that the patient usually captures and relates as the “the doctor’s way of being”. Goya's painting, Self-portrait with Dr Arrieta (1820), can be taken as an outstanding artistic representation of this expanded dimension of the medical act, capturing the “medical friendship” in action. Background: Chronic Pain (CP) has serious medical and social consequences, and leads to economic burden that threatens the sustainability of healthcare services. Thus, optimized management of pain tools to support CP patients in adjusting to their condition and improving quality of life is timely. Although Acceptance and Commitment Therapy (ACT) is considered an evidence-based psychological approach for CP, evidence for the efficacy of online-delivered ACT for CP is still scarce. At the same time, studies suggest that self-compassion mediates the change (...) in disability and psychopathological symptoms in ACT interventions for CP, although self-compassion is not a specific target in ACT. Thus, an explicit focus on self-compassion might increase the efficacy of ACT interventions for CP, although this hypothesis has not been tested. This study aims to develop an eHealth ACT and compassion-based self-management intervention for CP, the iACTwithPain, and to compare its efficacy in improving health outcomes to a similar ACT-only intervention and a waiting list group. Methods: The eHealth platform that will host the interventions will be developed using a flat design identity and will be interactive. The iACTwithPain intervention will comprise 8 weekly self-management sessions, and will be developed taking into consideration the psychological flexibility model applied to CP, with the addition of explicit compassion-based components. To analyse whether the iACTwithPain intervention will present superiority in improving CP’s impact and related health markers over the two other conditions, this study will follow a RCT design with three arms. CP patients will be recruited through direct contact with patient associations and healthcare services and a national press release in Portugal. Outcome measurement will be conducted at baseline, post-intervention and at 3- and 6-months follow-ups. The interventions’ acceptability will also be assessed. Discussion: The iACTwithPain intervention is expected to improve CP patients’ psychosocial functioning, quality of life, and empowerment, by promoting adaptive disease management and regulation of pain-related internal experiences. Results will contribute to a better understanding on the pertinence of adding compassion elements to ACT for CP and to reach an optimized intervention for CP. (shrink)

In this study, we consider the ways different degrees of dissent are established in interaction, especially in interactions among children. One important aspect in the development of the ability to argue is the framing of interactions as rather cooperative or agonistic. Different framings seem to allow for different forms of argumentative activity. The focus in this paper is on the mediation of degrees of dissensus in argumentation in child-child communication. This chapter focuses on one of the common fallacies in Western philosophy called 'poisoning the well' (PTW). There are some forms of ad hominem varieties that are usually a response to an interlocutor's claims. Unlike them, PTW occurs when we illegitimately prime our audience with a pre‐emptive strike against, or with adverse information about, an argumentative opponent before the latter has had a chance to say anything in her own defense, or in defense of her point of view. This has (...) the insidious effect of creating a conceptual framework according to which the audience – and maybe even the interlocutor herself – will interpret her claims as 'fulfilling' and 'confirming' the presumptions buried inside this conceptual trap. Most of the time Karl Popper's criterion of falsifiability is still quite a useful tool and one that can help us gain a firmer understanding of PTW. (shrink)

Generalized trust, the belief that most other people can be trusted, has positive consequences for health and wellbeing. An increased sense of community is often seen in times of crisis or disaster, but it is unclear whether this is the case in the COVID-19 pandemic. The objectives of the current study were to assess whether generalized trust increased in an early pandemic phase compared to pre-pandemic levels, and whether trust was lower in individuals who felt particularly threatened or burdened in (...) the pandemic. We compared levels of generalized trust in a population-representative Norwegian sample with pre-pandemic levels from the European Social Survey. Age- and gender-adjusted expected scores were compared to observed scores, using weighted data. Secondly, we tested whether indicators of pandemic-related strain, perceived health risks, or pandemic-related worry were associated with a lower level of generalized trust. This cross-sectional study was conducted in an early opening-up phase. The observed levels of generalized trust in an early pandemic phase did not differ significantly from expected levels based on pre-pandemic measures. Higher trust was found among individuals who reported personal experience with the COVID-19 disease. Pandemic-related worry and a high perceived health threat were both associated with a lower level of generalized trust. These results indicate that personal experiences with the COVID-19 pandemic could influence trust in others, although this link may be context-dependent. Generalized trust is considered to be an important asset in society, and promote health and well-being. As the pandemic evolves, there is a risk that we may lose, or a chance that we could gain, trust, with potential consequences for our health. (shrink)

The Coronavirus disease 2019 pandemic is affecting many areas of life and has led to major changes in undergraduate medical education. Even before the COVID-19 pandemic, high mental burden of medical students has frequently been reported in the literature. Additional pandemic-specific stressors could exacerbate this situation. This study aimed to assess mental health outcomes among medical students during the first semester after the COVID-19 outbreak and perception of the students on how the learning environment has changed. In May 2020, we (...) conducted a cross-sectional survey among undergraduate medical students at a large medical school in Germany. The survey included validated mental health instruments and self-developed items to examine the perception of the study situation during the COVID-19 pandemic. Open-ended questions were analyzed by conventional content analyses. The response rate was 59.2%. Overall, 61.9% of the students reported distress levels above the cutoff. Year 1 students reported significantly higher levels of distress, anxiety and depression than students during their second to fourth year of studies. 48.3% of the students indicated a decrease in their study motivation since the beginning of the COVID-19 pandemic with significant differences between study years. The binary logistic regression model showed that male gender, being in study year 2, higher distress scores and higher symptoms of depression were significantly associated with a higher likelihood for experiencing serious worries. In the open-ended questions on current concerns related to the impact of the COVID-19 pandemic on their studies, students most frequently reported concerns about missing relevant practical learning experience, difficulties with self-regulated learning and self motivation as well as study-related worries. Year 4 students reported significantly more worries about the lack of practical training than students from study years 1 to 3. Analysis of gender differences showed that female students reported more frequently diverse worries. In contrast, female students shared more frequently helpful strategies in all the categories compared to male students. Our findings suggest that medical students experience significant levels of distress and mental burden during the COVID-19 pandemic and highlight the need for ongoing psychological and educational support for medical students during the COVID-19 pandemic and after. Introduction: The use of human samples in genomic research has increased ethical debate about informed consent (IC) requirements and the information that subjects should receive regarding the results of the research. However, there are no quantitative data regarding researchers’ attitudes about these issues. Methods: We present the results of a survey of 104 US and 100 Spanish researchers who had published genomic epidemiology studies in 61 journals during 2006. Results: Researchers preferred a broader IC than the IC they had actually (...) obtained in their published papers. US authors were more likely than their Spanish colleagues to support obtaining a broad IC, covering either any future research project or any projects related to a group of diseases (67.6% vs 43%; adjusted OR = 4.84, 95% CI, 2.32 to 10.12). A slight majority of researchers (55.8%) supported informing participants about individual genomic results only if the reliability and clinical validity of the information had been established. Men were more likely than women to believe that patients should be informed of research results even if these conditions were not met (adjusted OR = 2.89, 95% CI = 1.46 to 5.72). Conclusions: This study provides evidence of a wide range of views among scientists regarding some controversial ethical issues related to genomic research, suggesting the need for more study, debate and education. In the interim, journals might consider including the investigators’ policies regarding these ethical issues in the papers they publish in the field of genomic epidemiology. (shrink)

This paper focuses on the scholastic technique of the Theravāda scholar-monk Chapaṭa Saddhammajotipāla. Chapaṭa is the author of several scholastic treatises in Pāli, the most voluminous of which is the Suttaniddesa, a commentary on the Pāli grammar of Kaccāyana. I offer a general introduction to the Pāli grammatical tradition and especially to the Pāli grammatical tradition of Burma, together with an introduction to the life and works of Chapaṭa. I also offer the first annotated translation of a passage from the (...) Suttaniddesa and in this way I show how the scholastic technique of Chapaṭa is based on a precise use of quotations, mainly from other Pāli or Sanskrit grammatical texts, but also from Buddhist literature. I finally give a preliminary assessment of typology and purpose of every different type of quotation. Although this paper consists mainly of preliminary work, it is the first essay entirely devoted to the Suttaniddesa, which is one of the most important grammatical works in the Theravāda tradition. El artículo rastrea en dos de los textos importantes de la obra de Niccolò Machiavelli : El Príncipe y los Discursos sobre la Primera Década de Tito Livio, elementos conceptuales inherentes a la noción psicoanalítica psicología de las masas. En el pensamiento del florentino es dable hallar cierta teoría psicológica, la cual es concebida como expresión de deseos y afectos humanos, a la cual el autor arriba a partir de una aguda observación empírica y del conocimiento del pasado. Machiavelli analiza (...) la ciudad como un campo de deseos en pugna, el deseo del pueblo de no ser oprimido y el de los nobles de ambición ilimitada. En el análisis que despliega el autor sobre estos deseos en conflicto, pero sobre todo en la relación entre el príncipe, los nobles y el pueblo, así como en la caracterización que realiza de éste, es posible hallar elementos en ciernes de la psicología de las masas desarrollos luego por el psicoanálisis freudiano. This document exposes the conflict from the Post-Rational cognitive perspective, understanding the conflict as a relational phenomenon, which emerges when the need of recognition is exposed to its contrary: the non-recognition. “To know oneself” means in fact, to recognize oneself through the mediation of the other. An individual develops himself by recognizing the “otherness” that constitutes him. The self that goes out toward the other and then returns as ipse/selfhood, having acquired self-awareness through the other. For this reason, recognition is (...) a fundamental element in the building of human subjectivity. Identity is a continuous process that lasts a lifetime. It contains a central nucleus that ensures continuity by which a person is able to recognize him/ herself. In this way individuals learn to define who they are through interactions with others, especially those whom are more significant. Being recognized by others and by the environment offer the individual a sense of unity and coherence about himself. Conflict arises when the other or the environment becomes a threat to the survival of the subjectivity; when the absence or lack of recognition leads to harm in a person. Relational experiences are able to engender and ensure recognition, while holding onto an indispensable acceptance of individual differences. They thus allow each one to bring forth their own unique identity. (shrink)

A lo largo de la historia, la filosofía ha situado gran parte del foco de su quehacer en el constructo intelectual de sistemas de pensamiento que profundizaran en el análisis del mundo. Si embargo, existe una tendencia marginal, dentro de la tradición filosófica, que apuesta por acercarse a la filosofía como una disciplina práctica. En este artículo trataremos de focalizar la investigación en dos pensadores, separados por más de dos mil años, que apostaron por situar la praxis en el epicentro (...) de la filosofía: Pirrón de Élide y Matthew Lipman. El primero dio lugar a un escepticismo donde la ejemplaridad del acto configuraba la doctrina. El segundo, Matthew Lipman, diseñó una metodología de filosofía para niños que ensalza la praxis comunitaria como eje desde el que fundamentar el aprendizaje de la filosofía. Cada uno, desde diferentes planos, el individual por parte de Pirrón y el colectivo en el caso de Lipman, han sido esenciales para poner el valor el sentido práctico de la filosofía. RESUMO O presente ensaio apresenta uma análise crítica das atuais democracias que se esvaziaram do poder deliberativo do demos para se tornarem, cada vez mais, democracias espetaculares. Inicialmente, seguindo os estudos de Foucault, analisam-se as implicações da governamentalidade sobre a democracia, principalmente a partir da emergência da opinião pública como técnica da razão de Estado. Posteriormente, relacionam-se os estudos de Foucault com as teses de Agamben a respeito da burocracia e a hierarquia, a fim de compreendermos como estas pesquisas desembocam (...) no conceito de democracias espetaculares defendido por Agamben. ABSTRACT This essay presents a critical analysis of the current democracies that have emptied themselves of the deliberative power of the demos to become, more and more, spectacular democracies. Initially, following Foucault's studies, we analyze the implications of governmentality on democracy, especially from the emergence of public opinion as a technique of reason of state. Later, Foucault's studies are related to Agamben's theses on bureaucracy and hierarchy, in order to understand how these researches lead to the concept of spectacular democracies defended by Agamben. There has been little research on the vocabulary of the Greek novelists. Gasda studied that of Chariton in the last century. He compared some of his terms with those of other authors and he concluded he should be placed in the sixth century A.D. Then Schmid considered that Chariton's language was not Atticist, and dated his novel in the second century or beginning of the third. In 1973 Chariton's language was studied by Papanikolaou. His research dealt above all with several (...) syntactic aspects and the use of some vocabulary, which led him to conclude that this language was closer to the koiné than that of the other novelists. But Papanikolaou went further in his conclusions: finding no trace of Atticism in Chariton, he considered him a pre-Atticist writer and, using extra-linguistic data, such as the citing of the Seres, the Chinese , placed him in the second half of the first century B.C. This chronology has been accepted by some, but already Giangrande has observed that this lack of Atticisms could have been intentional, in which case that date would be questionable. (shrink)
